- For any additional household occupants, fill out the names in the 'All Other Household Occupant(s)' section.
- Provide the main phone number for the occupants and a 'Work or Emergency Phone' if applicable.
- If there is a leasing agent involved, enter their name, address, and phone number in the designated area.
- Confirm whether tenants have received a copy of the community's CC&R's and Rules by marking 'Yes' or 'No'.
- Fill in the vehicle information, including 'Registered State', 'License Plate #', 'Year', 'Make & Model', and 'Color' for up to three vehicles.
- If there are more than three vehicles, list additional vehicles on the back of the form.
- Once all sections are filled, review the form for accuracy, then save, download, print, or share the completed form as needed.
