- If you have chosen an electronic payout method, fill in the full name of the account holder, the full name of the bank, and the branch name. Specify the account type as either current or savings.
- Provide your bank account number, along with the MICR code (mandatory for ECS mode) and IFSC code (mandatory for NEFT mode). Note that these codes can be obtained from your cheque book or bank.
- Acknowledge the responsibility of ensuring the correctness of the bank account number and other details provided in the form.
- Affix a Re 1/- revenue stamp and sign across the stamp. If the policy is assigned, include details of the assignor and assignee.
- Complete the acknowledgement section with the proposal/policy number, name of the policyholder, and the date of receipt.
- Finally, save your changes before downloading, printing, or sharing the completed form.
