- If you want to continue with that teacher, select 'Yes' or 'No' accordingly.
- After submitting the form, note that once you are assigned an instructor, they will contact you to schedule lessons at a convenient time.
- Be aware that there is a lab fee associated with both one and two credit private lessons.
- Once you have completed all the fields, ensure to save your changes, then download, print, or share the form as needed.
