- Confirm the specifications of the vehicle operated by the service member, indicating if it is equipped with an air brake or air-over-hydraulic braking system, and detailing the type of transmission.
- The commanding officer must certify the service member's experience by printing their name/rank, signing, and dating the verification section.
- Once all sections are completed, save your changes, and consider downloading, printing, or sharing the form for submission.
