12. If a time slot has been allotted for your request, enter the relevant dates.
13. Record the letter number issued by Nplindia for the time slot along with the date.
14. Choose the mode of collection for the certificate or report, indicating whether you prefer to collect it in person or have it sent by post.
15. Finally, include the name and signature of the depositor along with the date to certify your request.
16. Once you have filled out all sections of the form, you can save your changes, download the form, print it, or share it as needed.
