An Oregon notary acknowledgment form is a tool that notaries use to authenticate official documents. The form affirms that the signature(s) provided on the documents are indeed the signature(s) of the individual(s) and that they have signed the form with the mental capacity to do so.

To take an acknowledgment, the document signer must personally appear before you, the notary public, and declare that he or she has signed the document voluntarily. You should ensure that the signer understands the document and has not been coerced into signing.

On the certificate itself, the document's date, type, and the number of pages should be noted at the bottom of the certificate, e.g., “Attached to declaration, signed by John Hancock on July 4, 1776, two pages.” Write in the notarial journal, “used loose certificate” or “attached certificate.”

An Acknowledgement of Signature or Acknowledgement of Execution is a notary which verifies that a particular person signed a given document. It is often used for legal agreements, business documents, etc.

After witnessing the document being signed, the Notary then completes the appropriate certificate wording for the signature witnessing. An acknowledgment, on the other hand, does not require the Notary to witness the signature in most states.

Acknowledgment in an Individual Capacity - A statement by a person that he or she has executed an instrument for the purposes stated therein. The signer must personally appear, acknowledge that he or she willingly and knowingly signed the document, and the notary must identify him or her as the one who did indeed sign.
