- In Section 5, state how long the member will require the services (in weeks or months) and the frequency of visits per week or month.
- Section 6 requires you to indicate if there is a medical reason preventing the member from using public transportation. Describe the specific reasons.
- Section 7 asks whether a wheelchair van is needed and if an escort will accompany the member. Provide information on other transportation needs if necessary.
- Finally, in Section 8, obtain the required signature from the authorized provider and check the applicable title before entering the date.
- Once completed, save the changes, and you will have options to download, print, or share the form.
