- In part C, specify the amount to transfer — either the whole entitlement or a partial amount.
- In part D, identify the RRSP, RRIF, RPP, SPP, or DPSP to which the funds are being transferred. Fill in the necessary plan and individual numbers, as well as the employer's name and CRA registration number.
