This guide will walk you through the process of completing the application form online, ensuring that you understand each section and can submit your form accurately and efficiently.
Follow the steps to fill out the form correctly
- Press the ‘Get Form’ button to access the application for tuition waiver and open it in the online editor.
 - In the Employee Section, enter your full name, University telephone number, division/department, home telephone number, intramural address, student ID number, employee ID number, job title, and email address. Ensure all required fields marked with an asterisk are completed.
 - Select the term corresponding to the courses you wish to take: Fall, Winter, Spring, or Summer. Then, list up to two courses for full-time faculty and staff (one course for part-time faculty and staff) that you are requesting waivers for, including the course number, title, credit hours, start date, end date, and whether it is a graduate-level course.
 - Review the attached flow chart on page four to determine the tax implications for graduate courses. Indicate whether the graduate courses listed are taxable or not, checking the appropriate box and completing the Graduate Tuition Waiver/Reimbursement Tax Exemption Form if necessary.
 - Sign and date the application to acknowledge that you understand the terms of the tuition benefits. Make sure to keep a copy of the completed application for your records.
 - Submit the completed application (and tax exemption form if applicable) to your supervisor or department head for their review and signature.
 - Once your supervisor or department head has signed and dated the application, return it to the Office of Total Rewards. It is advisable to submit the application before the course start date. Completed applications must be received within 30 days after the course begins.
 - Allow the Office of Total Rewards two weeks to process your application. Upon confirmation of eligibility, they will notify the Bursar’s Office regarding the tuition waiver.
 - Once you have successfully completed the course(s), the Bursar’s Office will post the actual tuition credit to your student account.
 - For any taxable tuition benefits, the applicable taxes will be deducted from your paycheck as approved by the Office of Total Rewards.
