6. Provide income information by listing each employer and the amount earned in 2017. Ensure to submit W-2s if applicable. Attach a separate page if more space is needed.
7. Review the completed form. Make sure all sections are completed fully without any blank fields. Confirm that all required tax information is included.
8. Sign the worksheet in the designated areas. The student and applicable parents must sign and date the form. Do not sign electronically.
