- Fill in Block 7 with the Importer Mailing Address. This can be either a P.O. box or a street address, limited to two lines with a maximum of 32 characters each.
- Continue with Block 8, inserting the city of the mailing address, followed by the State Code in Block 9, and the ZIP Code in Block 10.
- For foreign mailing addresses, complete Block 11 with the appropriate Country ISO Code.
- If the physical location address is different from the mailing address, provide it in Block 12. Include the city in Block 13, State Code in Block 14, and ZIP Code in Block 15.
- In Block 16, if the address is foreign, insert the Country ISO Code.
