- For existing users, enter their user logon IDs and indicate whether you want to add, change, or delete access by checking the appropriate box. Specify the functions needed, such as payment request or general inquiry.
- Section III requires the authorizing official’s signature. Ensure that the individual signing certifies that all required information is correct and that signed copies of the User Responsibility Statement are maintained.
- After completing all sections, review the filled-out form for accuracy. Save changes, and then you have the option to download, print, or share the form as needed.
