- Next, fill out the 'Change in Case Status.' For each child, specify the effective date of change, whether the case is active or closed, and provide the effective date of closing and reason for closing if applicable.
- Complete the 'Change in Address' section. Enter the effective date of the change, the current family address, and the new family address with all required details.
- For 'Change in Adoption Status,' provide the effective date, details of the adoption assistance agreement for each child, and indicate the status of the final adoption decree.
- Lastly, ensure to address the distribution instructions. Prepare the original and two copies of the completed form, with the reporting state retaining the original, the recipient state retaining one copy, and the adoptive parents receiving another.
