How to fill out the Clackamas County Name Change online
This guide provides clear and supportive instructions for users seeking to complete the Clackamas County Name Change forms online. Whether you are transitioning to a new name or simply seeking a change, these steps will guide you through the process with ease.
Follow the steps to complete the name change process efficiently.
- Press the 'Get Form' button to access and display the form in your digital workspace.
- Begin by completing Form 01, titled 'Petition for Change of Name of an Adult.' Ensure every field is filled out except for the case number, which will be assigned later. Sign the petition where indicated.
- Next, complete Form 02, labeled 'Notice of Petition for Change of Name of an Adult.' For the deadline to file objections, enter a judicial day at least 15 days after you post the notice. Input this same date in the section that requests, 'Do not remove this notice before ____.'
- Submit both completed forms to the Civil Case Unit in Room 104. The staff will verify the documents and assign a case number. At this point, pay the filing fee, which is currently $105.00.
- Upon receiving your case number, post the Notice on the courthouse bulletin board in the designated area for public notices. You may have another individual assist with this posting, but they must also remove the notice later and confirm posting.
- If no objections are filed, return to the courthouse at least fifteen days later to proceed with the next steps. Remove the Notice from the bulletin board.
- Complete Form 03, 'Proof of Posting of Petition for Name Change of an Adult,' but do not sign it yet. Then, prepare Form 04, 'General Judgment of Name Change of an Adult,' ensuring to leave three sections blank for the judge's signature.
- Return to the Civil Case Unit window with Forms 02, 03, and 04. You may sign Form 03 at the window. Subsequent to review, your documents will be forwarded to the judge for processing.
- After the judgment is signed, complete Form 05, 'Notice of General Judgment of Name Change of an Adult.' Post this notice on the bulletin board in the designated area for second postings. Another individual can assist with this task.
- At least fifteen days later, return to the courthouse to remove the posted Form 05. Complete Form 06, 'Proof of Posting Notice of General Judgment of Name Change of an Adult,' without signing it.
- Take both Forms 05 and 06 back to the Civil Case Unit window for review. You can sign Form 06 at the window. This form must be interviewed by the person who posted and later removed the notice to validate the process.
- Once all steps are correctly followed, the clerk will mail you a certified copy of the judgment signed by the judge, which serves as the legal document for your name change.
- To update your name on other official records, you must use the certified copy of Form 04. You are responsible for notifying other agencies to reflect your name change.
- Remember that while these forms work for most people, they may not cover every unique situation. Consult an attorney if you have questions or if the instructions are unclear.
