  8. Return to the Civil Case Unit window with Forms 02, 03, and 04. You may sign Form 03 at the window. Subsequent to review, your documents will be forwarded to the judge for processing.
  9. After the judgment is signed, complete Form 05, 'Notice of General Judgment of Name Change of an Adult.' Post this notice on the bulletin board in the designated area for second postings. Another individual can assist with this task.
  10. At least fifteen days later, return to the courthouse to remove the posted Form 05. Complete Form 06, 'Proof of Posting Notice of General Judgment of Name Change of an Adult,' without signing it.
  11. Take both Forms 05 and 06 back to the Civil Case Unit window for review. You can sign Form 06 at the window. This form must be interviewed by the person who posted and later removed the notice to validate the process.
  12. Once all steps are correctly followed, the clerk will mail you a certified copy of the judgment signed by the judge, which serves as the legal document for your name change.
  13. To update your name on other official records, you must use the certified copy of Form 04. You are responsible for notifying other agencies to reflect your name change.
