Follow the steps to complete the Relocation Form online.
- Press the ‘Get Form’ button to access the Relocation Form and open it in your preferred editor.
- Begin by filling in the facility information section. Include your facility name as found on the first page of your DEQ permit or Permit by Rule (PBR), followed by your facility ID number, mailing address (including street, city, state, and zip), facility contact phone, the facility contact's name and title, name and title of the responsible official, and lastly, obtain the responsible official's signature and date.
- Next, move to the equipment section. Indicate the type of equipment—such as concrete batch, rock crusher, hot mix asphalt, or if applicable, select 'Other' and provide a description.
- For the permit section, specify the type of permit—either Permit to Construct (PTC), Operating Permit (OP), or 'None.' Make sure to note the corresponding permit or PBR number and the date issued.
- In the current location section, complete the details including the current Idaho county, nearest city, GPS coordinates in WGSG4 format, and a description of the current location. Indicate whether the plant is in storage/idle, out of state, or provide an alternative description. Also, mention how long the plant has operated at this location.
- Proceed to the new location section where you will enter similar details for the new Idaho county, nearest city, GPS coordinates, and describe the new location accordingly. Again, select if the status is in storage/idle, out of state, or provide an alternative description. Also, include the estimated start and end dates of the operation at the new location.
- Finally, once all sections are accurately filled out, you can save your changes. You have the option to download, print, or share the completed form. Ensure you follow the submission instructions provided in the form to complete the process.
