6. If you have read and signed Form 1264-3, sign and date the form. If not, ensure you read, sign, and submit it to your IT Security Manager first.
7. In the Login Access Detail section, check the 'Add User Account' box if you need to add an account. If you need to modify a user account, select the 'Modify User Account' box, and specify the effective date.
8. List each software application under the corresponding section and indicate the desired permissions for each one.
9. If you need to remove a user account, check the 'Remove User Account' box. If this applies to all accounts, mark 'Remove from All Accounts' and provide the effective date as needed.
10. Ensure all necessary signatures are collected from the BLM Supervisor/Task Manager, Local IT Security Manager, and State Application/Program Representative.
11. Fax the completed form to L&RPO at 303-236-6691.
