- Enter the patient details including their Medicare number, patient’s reference number, first name, surname, address, and postcode.
- Specify the allied health professional to whom the patient is referred by writing their name or type of professional. Then, fill in their address and postcode.
- Ensure you sign and date the form in the designated section for the referring GP's signature.
- In Part B, the allied health professional who services the patient must complete their section. This includes providing their HIC provider/registration number, name, signing the form, and dating it after services are rendered.
- If multiple services are provided, photocopies of the form should be made for each service prior to signing. Sign the original copy after the first service and each photocopied form after subsequent services.
- Finally, retain copies of all signed forms for record-keeping and submit originals to Medicare for payment.
