3. In the Meningitis Response section, read the provided link regarding Meningococcal Disease. Select one of the boxes to indicate your vaccination status, and be sure to sign and date the form. If you have received the vaccination, include the date of the shot.
4. Proceed to Section A for proof of immunity to Measles, Mumps, and Rubella. You have three options to complete this section: documentation of vaccines, serological titers, or an immunization record. Ensure that the proper documentation is included, especially if completing Section B or C.
5. For Section B, list the dates of the first and second doses of the Measles, Mumps, and Rubella vaccinations, ensuring that each vaccine is received at least 30 days apart, starting after the student’s first birthday.
6. If the student is under 18, obtain a parent’s signature in Section D. Ensure that sections are completed and signed by a health care provider or school official.
7. For Section C, if using serological titers, include lab results indicating immunity along with a signed lab report from a health care provider.
8. In Section D, the health care provider or school official must provide their signature, stamp, seal, or attach their business card verifying the completion of Sections A, B, or C.
9. Review the entire form for accuracy. Once verified, save your changes, then download or print the completed form as needed.
10. Submit the completed health form via email to health@sva.edu with your SVA Student ID number in the subject line, or fax it to 212.592.2216. Be sure to allow up to 3 business days for processing.
