- Line 4 will be calculated by multiplying the values in lines 2 and 3, with line 3 expressed as dollars per hundred.
- The tax collector will date and sign the form upon completion. Make sure to keep a copy for your records.
- If there are no new constructions or assessments, enter 'NONE' in the relevant sections and ensure the form is still signed and filed.
