- Bring the completed form to the international office at your current school for the DSO/RO to complete Section 2.
- Have your DSO/RO sign and date the form and include any necessary information regarding your status or OPT dates.
- Email the completed form and the subject line ‘SEVIS Release Form’ along with your name as it appears on your passport to newstudent@ic.uci.edu.
- Upload a copy of your passport's biographical page using the passport upload tool as indicated on the Uci International Center's document upload resources.
