- Fill out the details of the securities including type, face value, and the corresponding time frames for any lock-in period.
- If necessary, attach an additional annexure if the space provided in the form is insufficient for listing certificate numbers or distinctive numbers.
- Review the statement confirming that the original certificates are free from any charge. Each holder must sign in the signature fields provided.
- Once all information is accurately entered, save, download, print, or share the completed form as needed.
