Use this form to authorise another person to apply on your behalf under the Government Information. (Public Access) Act 2009 (GIPA Act) for access to your personal information held by the Department.

MC authority is a number assigned by the FMCSA to identify a carrier operating for hire and transporting regulated commodities in interstate commerce. This MC number is also known as an operating authority or trucking authority.

The Federal Motor Carrier Safety Administration (FMCSA) was established within the Department of Transportation on January 1, 2000, pursuant to the Motor Carrier Safety Improvement Act of 1999 (49 U.S.C. 113).
