How to fill out the Home Inspection Checklist online
Filling out the Home Inspection Checklist online is an important process that ensures the accurate assessment of facilities for veterans’ care. This guide will provide you with step-by-step instructions to complete the form efficiently and effectively.
Follow the steps to complete your Home Inspection Checklist.
- Click ‘Get Form’ button to obtain the form and open it in the designated editor.
- Complete Part 1 by noting the numbers of operating beds, the beds authorized for VA per diem payments, and the patient census including both veterans and non-veterans. Also, include the staff positions authorized and actual staff available at the time of the inspection for each care level provided by the home, such as nursing home, domiciliary, and/or hospital.
- In Part II, enumerate the total staff positions for the facility. Break down the assigned full-time equivalent (FTEE) for each level of care. For instance, if there are 12 registered nurses, specify the breakdown across the different care levels.
- Progress to Parts III through V, which involve documenting the nursing staffing patterns for each level of care. Provide details regarding the average number and type of nursing staff on each shift for a selected 4-week period.
- In Parts III, IV, and V, calculate the average nursing care hours per patient, per day. Use the provided formula: Nursing Care hours/patient/day = (Total staff in average week X 8 hrs) ÷ (Patient census (veteran + non-veteran) X 7 days). Ensure to follow the instructions regarding meal times.
- Once all sections are completed, review the information for accuracy. After verification, you can save your changes, download, print, or share the completed form as needed.
