- Part 7: About payment of your Maternity Allowance. Choose whether you would like your payments to be made directly into your bank account or via weekly cheques at a Post Office, and provide the necessary details.
- Part 8: Maternity Certificate. Ensure you attach your Maternity Certificate (Mat B1), and tick the box indicating if it is included.
- Part 9: Other information. Use this space to provide any additional information that may support your claim.
- Part 10: Your declaration. Read the declaration carefully, then sign and date the form. If someone else filled out the form on your behalf, indicate this.
