Follow the steps to complete your Audiogram online.

- Complete the required fields at the top of the form: enter your family name, given name, and any other names in the designated sections. Indicate your nation and sport.
- Select your gender by marking either male or female. Provide details about the event, choose from regional championships, World Championships, or Deaflympics.
- Input your date of birth in the day/month/year format as required.
- In the audiogram section, enter the audiometer name and examiner's name. Specify the calibration method used by choosing from ANSI 1969 or ISO 1964, or others if applicable.

- Calculate and document the pure tone average by averaging the results of air conduction testing at the specified frequencies.
- Identify the type of hearing loss by checking the appropriate box for each ear, and provide any relevant comments in English as needed.
- Finalize your form by ensuring all required fields are filled. Save your changes, download, print, or share the Audiogram as needed.
