- Fill out the employee’s work schedule for each day of the week by entering the working hours. If the schedule varies, note the current schedule and provide an explanation on the back.
- Indicate the Number of HOURS the employee works each week and the Number of DAYS they work each week.
- Enter the Hourly Rate, Weekly Gross, and any additional income such as Tips or Bonus. Make sure to specify how often the employee is paid by circling one of the options provided.
- If the employee has experienced any lapses in employment, indicate Yes or No, and provide the necessary dates if applicable.
- If the employee has worked a different schedule in the past, provide that schedule in the specified format, just like the current one.
- Provide a brief description of the type of work the employee is engaged in within the designated area.
- Complete the Employer section by providing the name of the Business, phone number, and address, then have the employer sign and print their name, along with their title and date.
- Finally, ensure the document is ready for submission. You can then save changes, download, print, or share the completed form as necessary.
