- Complete 'Part IV: Hospital Declaration' by ensuring the hospital authority certifies your NHIF contribution card is valid. They will enter their name, designation, and signature along with the official hospital rubber stamp.
- Attach a copy of the discharge summary, which must carry the hospital seal. Enter the disease code (ICD10) if applicable.
- Finally, review all provided information for accuracy, save your changes, and follow the prompts to download, print, or share your form as needed.
