How to fill out the NIHSS online
The NIH Stroke Scale (NIHSS) is a valuable tool used to assess the severity of a stroke. This guide provides clear instructions on how to complete the NIHSS form online effectively, ensuring accurate assessment and documentation.
Follow the steps to complete the NIHSS online with ease.
- Press the ‘Get Form’ button to access the NIHSS form and display it in your online editor.
- Begin by answering the first section, which assesses the level of consciousness. You will need to select the appropriate score based on the patient's responsiveness.
- Proceed to the 'LOC Questions' section. Here, ask the patient about the current month and their age to evaluate their cognitive orientation.
- Next, address the 'LOC Commands' area. Ask the patient to open and close their eyes and request them to grip and release their nonparetic hand.
- In the 'Best Gaze' section, evaluate the patient's ability to perform horizontal eye movements, and select the score accordingly.
- Move to the 'Visual' section, where you will assess the visual fields by checking both the upper and lower quadrants.
- Assess the 'Facial Palsy' by encouraging the patient to show their teeth, raise their eyebrows, or close their eyes.
- Continue to the 'Motor Arm and Leg' sections (5 and 6). Position the patient's right and left arms, as well as right and left legs, and document the appropriate scores.
- Evaluate limb ataxia by finding evidence of a unilateral cerebellar lesion and selecting the corresponding score.
- In the 'Sensory' section, test the patient’s sensation with a pinprick and select the relevant score.
- Review the 'Best Language' section where prior examination sections will guide your scoring on language comprehension.
- Assess 'Dysarthria' by collecting a speech sample from the patient, asking them to read or repeat words.
- Finally, address 'Extinction and Inattention' (formerly Neglect) to gather information about any neglect that may have been indicated in prior tests.
- Once all sections are completed and all scores are selected, check the total NIHSS score at the end of the form.
- You can now save your changes, download the completed document, print, or share it as needed.

The NIHSS score at admission serves as a baseline for evaluating a stroke patient's condition upon entering the hospital. This initial score helps healthcare providers understand the stroke's impact on functioning. A higher score typically indicates a more severe stroke, prompting immediate medical attention. Tracking changes in the NIHSS score can guide recovery strategies throughout treatment.
