- For retirees, provide a Certification signed by the Chapter President. If they are unavailable, an alternative certification from the Barangay Chairman must be submitted.
- Complete the Duly Accomplished Information Sheet for all beneficiaries. If applying under specific plans, only one beneficiary needs to submit this form as the claimant.
- Attach photocopies of two valid IDs that include signatures for all beneficiaries, ensuring the signatures match those on the Information Sheet.
- For specific claims, such as those involving minor beneficiaries, ensure to submit the necessary guardianship documents.
