7. Move to section 2, where you will provide the central office location details such as state, city, zone, and address (fields 2.1 to 2.6). If applicable, mention the number of additional locations in field 2.11.
8. In section 3, enter worker information including total number of employees, gender breakdown, foreigners, and any specific categories like retirees or individuals with different abilities (fields 3.1 to 3.16).
9. Review the declaration in the final section. Record the name and ID of the employer or legal representative, as well as signatures as required in sections A to F, where some fields may be filled by MTEPS staff.
