Effective Jan. 1, 2024, OSHA issued a new rule requiring establishments with 100 or more employees in designated industries with high injury and illness rates to electronically submit the foregoing information to OSHA once a year through the agency's new Injury Tracking Application (ITA).

Employers in California that have establishments meeting one of the requirements below are required annually to electronically submit Form 300A injury and illness data: All establishments with 250 or more employees, unless specifically exempted by section 14300.2 of title 8 of the California Code of Regulations.

Covered establishments must submit their annual 300A, 300, and 301 data to the Injury Tracking Application (ITA). The records must be maintained at the worksite for at least five years. Each February through April, employers must post a summary of the injuries and illnesses recorded the previous year. Also, if requested, copies of the records must be provided to current and former employees, or their representatives.

Employers must report work-related fatalities within 8 hours of finding out about them. Employers only have to report fatalities that occurred within 30 days of a work-related incident. For any inpatient hospitalization, amputation, or eye loss employers must report the incident within 24 hours of learning about it.

Electronic Submission of Records Establishments that meet certain size and industry criteria are required to electronically submit injury and illness data from their OSHA Form 300A, 300, and 301 (or equivalent forms) once per year to OSHA.

What to include in a work incident report The date and time of the incident. The name of the witness or author of the report. A detailed description of the events. The names of the affected parties. Other witness statements or important information. The result of the incident.

To be effective, an incident report should include: Incident Details: A clear description of what happened, including time, location, and context. People Involved: Information on everyone present or affected by the incident. Impact: The consequences of the incident, such as injuries or property damage.

Your statement should reflect what happened, without adding any personal opinions or assumptions. It is also important to document any injuries sustained, the cause of the incident, and any actions that were taken to prevent it from occurring again.
