A copyright assignment is the transfer of copyright ownership rights from one party to another. This transfer is not valid unless it is in writing and signed by the owner or its authorized agent. If you want to transfer a right on a non-exclusive basis, a written agreement is usually not required.

The last part is optional, although it's encouraged for clarity and completeness.

Describe the proposed use of the copyrighted material. If necessary, attach a copy of the quotations, diagrams, pictures, and other materials. If the proposed use is extensive, describe it in broad terms. Eliminate any ambiguities and be sure that your request for permission covers the full scope of your needs.

Your words/illustrations/creations used to bring an Idea to life are considered the copyrightable expression. This is why, in general, blank forms are not protectable under copyright law whereas completed-filled-in forms would be protectable.
