An adult child of the patient. Any parent, whether adult or minor, for his child. The patient's sibling. The patient's other ascendants or descendants.

The person who assigns power of attorney is known as the principal, and the person to whom the principal gives POA is the agent. The principal — that is, the person who assigned POA to someone (an agent) on their behalf — can revoke it.

Adults. In most states, the default surrogate decision maker for adults is normally the next of kin, specified in a priority order by state statute, typically starting with the person's spouse or domestic partner, then an adult child, a parent, a sibling, and then possibly other relatives.

If not, your legal next of kin have the right to make decisions for you. Ohio recognizes this order of your decision makers: legal guardian (if applicable), spouse, majority of adult children, parents, majority of adult siblings, or other nearest relative.

Ohio law recognizes an Order of Decision Makers if you are unable to make health care decisions for yourself. If the state has appointed a guardian, this person is the first decision maker. If not, your legal next of kin have the right to make decisions for you.

Living will. A living will is a written, legal document that spells out medical treatments you would and would not want to be used to keep you alive, as well as your preferences for other medical decisions, such as pain management or organ donation. In determining your wishes, think about your values.

Advance directives have three components: a living will, a healthcare power of attorney, and a Do Not Resuscitate (DNR) Order. A living will tells doctors how the patient wants to be treated if they are unable to make decisions on their own.

In Ohio, these are the requirements to make the following documents valid: Trust-Signed at the end by the Settlor and, typically, the Settlor's signature is witnessed and signed by two (2) disinterested witnesses and/or executed by the Settlor in the presence of a Notary Public.

In the few other states where handwritten wills are still valid, often they will require notarization. That is not the case in Ohio. While a notary does give a certain amount of legitimacy to handwritten wills, there's no state statute saying it's required.
