Dear Sir or Madam: I am a victim of identity theft. I recently learned that my personal information was used to open an account at your company. I did not open or authorize this account, and I therefore request that it be closed immediately.

You may not be aware of it, but you can lock your Social Security number to prevent unauthorized access if your SSN has been compromised. To block electronic access to your SSN, call the Social Security Administration at 800-772-1213.

Do not routinely carry your SSN. Never say your SSN aloud in public. Beware of phishing scams (emails, internet links, and phone calls) trying to trick you into revealing personal information. You should never provide your SSN to someone you don't know who calls you on the phone and requests it. This same warning applies to unsolicited emails and any forms you fill out on the Internet. In general, don't give your SSN to anyone unless you are absolutely certain they have a reason and a right to have it.
