To edit a SOAP Note, navigate to your: Personal Profile. Select SOAP Notes. Type in Search Clients to find and select a Client to open their history. Locate and select the SOAP Note to open the current version. Select Edit to activate the fields and tools. Make changes as needed.

For HIPAA purposes, a SOAP Note that has been saved or saved as a draft cannot be deleted. A copy of each saved note is stored in the SOAP Notes tab of the Client's record for viewing and editing.

You can delete a specific note from the database table 'pnotes'. Once deleted there, it will never reappear.

Edit a signed SOAP or Simple Note An encounter cannot be edited or deleted after it has been signed. The act of signing a chart note renders the note a legal document. However, you can add an amendment/addendum to the note.

ubjective The section is the place to report anything the client says or feels that is relevant to their session or case. This includes any report of limitations, concerns, and problems. Often living situations and personal history (ex. PMH or Occupational Profile) are also included in the section.

Medications or diagnoses associated with the encounter will continue to be accessible on the patient's Summary page but all other information associated with the encounter, including vitals recorded in the flowsheets, will not be retrievable. In the case you have already signed a note, it can no longer be deleted.

From the Encounters list (see Viewing all encounters for a patient), locate the encounter you want to delete and click the trash can icon in the Actions column. A confirmation pop-up appears; click Okay, and the encounter is deleted. From an open encounter, in the top-right corner, click Delete.
