If you've manually applied formatting to any of your titles, they might show up in the TOC. But a title in a document with a "Title" heading (or "Subtitle" heading) generally won't appear in the TOC (or outline). Instead, use only Headings 1-6 for your titles.

Go to References > Table of Contents > Custom Table of Contents. Select Modify. If Modify is grayed out, change Formats to From template. In the Styles list, click the level that you want to change and then click Modify.

That is a "Content Control." Use the Properties to fix this. You can right-click on the control to change the properties. Otherwise, you can use the Properties button on the Developer Tab to reach them. Uncheck the one that says it can't be deleted.

Check the screen to ensure the correct headings are shown. Do these match the heading styles you have applied. Also check if the TOC is set to display enough levels of headings. Increase the levels by clicking on the drop down list.

To customize your existing table of contents: Go to References > Table of Contents. Select Custom table of contents. The TOC field may have been locked. Try selecting the Table of Contents and then press CTRL+SHIFT+F11 (unlock field) and see if you can then update the Table of Contents. Another possibility is that the Table of Contents has been unlinked from the data in the document.
