You group radio buttons by drawing them inside a container such as a Panel control, a GroupBox control, or a form. All radio buttons that are added directly to a form become one group. To add separate groups, you must place them inside panels or group boxes.

Essentially, you can use the class name "form-control" on every other form element type except the radio button input type. Using that class "form-control" on a radio button tag makes it disappear therefore just remove the class attribute and the radio button will appear.
