After creating your check-box, right-click it (in Form Edit mode) and select Place Multiple Fields. This will allow you to easily create multiple (unique) copies of that box on the page.

Click and drag the mouse until the form field is of the desired size. Double click on the page to create a form field of the default size. The Radio name-value entry box appears. In the Radio Group Name text box, type a name that identifies the radio button.

Select the grouped object and press Ctrl+Shift+G. Right-click the grouped object, scroll to Group, and select Ungroup. Select the grouped object. Go to the Home tab, click Arrange, and select Ungroup.
