The correct format to be used in all Government issued documents are First name(Given name) Middle name followed by Last name (family name or father's name). Always expand your initials and mention your full name in all the documents else you will end up with different names on different documents.

As far as I'm aware, in the UK, post-nominal letters would be listed by university degree (in ascending order), followed by membership of learned societies, regardless of how accreditation ot this society was achieved. So, yours would be Firstname Lastname, BSc (Hons), MSc, MBPsS.

The diploma (as a document certifying a qualification) may also be called a testamur, Latin for "we testify" or "certify" (testari), so called from the word with which the certificate begins; this is commonly used in Australia to refer to the document certifying the award of a degree.

No statewide policy. In California, public colleges have been using a range of policies about what names they record on diplomas and in other records, as no statewide policy existed.

The institution will usually use whatever name they have for you on their system. If you only have your name listed as first name and surname, that's all they'll print. And they won't reprint a diploma with a different name, so if you want your middle name included, make sure they it's listed on their system.

You should use whichever name you would prefer to see on the diploma when you look at it. If you like your middle name, then by all means include your full middle name. If you don't like your middle name, then use your middle initial.
