On the Design tab, select Watermark. In the Insert Watermark dialog, select Text and either type your own watermark text or select one, like DRAFT, from the list.

Try using a private browsing window (i.e. Incognito mode in Chrome) and ensure you are logged-in with only one Google account in that window. If you only see the "Help" menu option, and none of the other Certify'em menu options, reload your Google Form, wait 30-40 seconds after it loads, then try again.
