The TOC field may have been locked. Try selecting the Table of Contents and then press CTRL+SHIFT+F11 (unlock field) and see if you can then update the Table of Contents.

If you've manually applied formatting to any of your titles, they might show up in the TOC. But a title in a document with a "Title" heading (or "Subtitle" heading) generally won't appear in the TOC (or outline). Instead, use only Headings 1-6 for your titles.

Another approach is to use the Go To feature: Just press F5 to display the Go To tab of the Find and Replace dialog box, choose Field at the left side, and then enter "toc" (without the quote marks) at the right side. When you press Enter, Word jumps to the TOC.
