In Microsoft Forms, open the form you want to edit. Select the question to which you want to add a picture. Select Insert media (picture icon) on the right side of the question. In the Insert media pane, select Image.

You cannot digitally submit a photo to us. You must provide a physical photo with your application. After you apply, a U.S. Department of State employee will review your photo and decide if we can accept your photo.

To insert an image into your form, you will need to add a Section or Description Area Field to your form and then click on the Insert Image button (which looks like a picture of mountains). How do you self attest a picture? Self attested photograph means you have to attest it yourself instead of a gazetted officer. Get the photo and after pasting it on the document or form put your signature in running hand partly on the base document and partly on the photo.

Go to the Photo Upload section in the 3rd page of the application. Go over the photo upload requirements to make sure your photo complies with all listed guidelines (certain countries may have additional requirements) Click the Choose FIle button and select the appropriate photo from your computer.
