(Reuters Health) - Nearly 37 percent of Americans have advanced directives for end-of-life care if they become seriously ill or unable to make health care decisions, according to a new analysis of recent research.

The qualitative analysis identified four barriers to ACP: 1) limited patients' participation in autonomous decision making, 2) cognitive and emotional barriers to discussion, 3) lack of readiness and awareness of early discussion, and 4) unprepared healthcare professionals and healthcare system.

The primary outcome is completion of four key ACP behaviours: identification of a surrogate decision maker, communication about goals, completing advance directives and ensuring documents are in the medical record.

Caregivers identified 2 barriers to having advance planning discussions: (1) passive avoidance and (2) active avoidance. These barriers often overlapped with all 3 types of planning discussions (Fig. 1).

A further critical limitation of advance care plans is that even when they are documented, they are not accessible in practice, and where they are available, health care professionals and family members do not always follow the documented preferences.
