Healthcare advance directives should state both what you do want and what you don't want.You retain the right to override the decisions or your representative, change the terms of your living will or POA, or completely revoke an advance directive.

Euthanasia is sometimes called aid-in-dying, mercy killing, death with dignity, etc. An advance directive is a document by which a person makes provisions for health care decisions in the event that, in the future, he/she becomes unable to make those decisions.

You can change your directives at any time. If you want to make changes, you must create a new form, distribute new copies and destroy all old copies. Specific requirements for changing directives may vary by state.

An advance directive isn't the same as a DNR DNRs, like POLST forms, require a physician's sign-off and are medical orders, instructing emergency personnel to withhold resuscitation efforts. If you don't want to receive resuscitation, including that information in an advance care directive is not enough.

A prehospital DNR order is generally a simple, one-page document; you don't need a lawyer to prepare it. You do, however, need to talk to your physician, who will sign your DNR. In some states, adult witnesses or a notary public must also watch you sign the order.

Advance directives have no expiration date, so review them every five or 10 years and revise them if your situation or your health changes.

All valid Durable Powers of Attorney for Health Care (DPAHC) and Natural Death Act Declarations remain valid. Thus, unless your existing DPAHC has expired you do not have to complete a new Advance Health Care Directive. A DPAHC executed before 1992 has expired and should be replaced.
