Euthanasia is sometimes called aid-in-dying, mercy killing, death with dignity, etc. An advance directive is a document by which a person makes provisions for health care decisions in the event that, in the future, he/she becomes unable to make those decisions.

A prehospital DNR order is generally a simple, one-page document; you don't need a lawyer to prepare it. You do, however, need to talk to your physician, who will sign your DNR. In some states, adult witnesses or a notary public must also watch you sign the order.

Some experts suggest that you either make a living will or assign a health care proxy. If you choose to have both, naming a health care proxy should be a priority, to ensure that someone can act in situations not covered in a living will.

Get the living will and medical power of attorney forms for your state, or use a universal form that has been approved by many states. Choose your health care agent. Fill out the forms, and have them witnessed as your state requires.
