With an alphabetized (by description) list of existing codes to assist in locating the most appropriate code for the operative report at hand. If an appropriate code could not be found, the clerk was expected to create a new code with appropriate attributes. This practice led to uncontrolled growth in the code tables. Clinical researchers often need to select a patientpopulation for a retrospective study orpractice analysis. At our clinic we were faced with an outdated indeingsystem that limit.

Medical record indexing is the categorization of medical records based on specific criteria such as patient identification, diagnosis, or treatment.

The master patient index (MPI) is a valuable reference for basic demographic information and resident activity (i.e. admission and discharge dates) within one source. The MPI is an index maintained separately from the resident's medical record.

The practice of engaging the patient in identifying themselves and using two patient identifiers (full name, date of birth and/or medical ID number) is essential in improving the reliability of the patient identification process.

A unique patient identifier (UPI) is a method for standardizing patient identification. Individuals are assigned a unique code, and that code, rather than a Social Security Number, name, or address, is what is used by healthcare organizations to identify and manage patient information.

Medical records contain a wide range of information about a patient's health history, such as their medical diagnoses, treatments, medications, etc. Indexing involves assigning keywords or codes to specific data points within the medical record to make it easier to search and retrieve information.

Acceptable identifiers may be the individual's name, an assigned identification number, telephone number, date of birth or other person-specific identifier." Use of a room number would NOT be considered an example of a unique patient identifier.

The MPI generates a unique patient ID, called an EUID, for each enterprise record. The MPI Enterprise Record consists of one or more facility records. The best demographic information for the patient will be used to create the Single Best Record.
