Follow these directions to submit an SRA: Log in to Student Center. Click on the Self-Reported Absence link in the bottom right corner, under “Student Services Links” When completing the form, make sure you include each course and assignment or test you will miss during your absence.

Generally, these are the steps you'll take: Identify your applicant type. Narrow down your program choices. Submit your application on OUAC. Create your Student Center account. Mark all deadlines on your calendar. Submit grades. Submit supplementary forms. Wait to receive your admission decision.

You must have a minimum overall average of 75% (3.0 GPA) in the last 10 full courses you completed at university.

Official Final Transcript(s) Please have your institution email official electronic transcripts to gradapps@uwo.ca. In cases where final transcripts were used to apply to the program, only the official copies of the final transcripts are required.

Self-Reported Absence (SRA) - Discontinued September 2022 Self-Reported Absences will no longer be available to students as of September 2022. Please stay tuned for updates on a new absence reporting system. SRAs are for absences up to 48 hours for course work that is worth 30% or less of your final grade.

There is a $150.00 application fee (payable through the online application process). Unfortunately, we cannot grant waivers for the application fee.
