- Record the Oregon taxable miles in Column I based on the miles driven on Oregon public roads.
- Enter the number of axles in Column J for declared weights above 80,000 pounds, and input the appropriate tax rate in Column K.
- Calculate the Oregon Highway Use Tax in Column L by multiplying the taxable miles by the tax rate.
- Proceed to Section 2 and fill Box 1 with the total from Column L, then Box 2 with any Oregon state fuel tax paid during the reporting month along with supporting invoices.
- Input other credits in Box 3 and calculate the total due in Box 4 by subtracting Boxes 2 and 3 from Box 1.
- If filing late, calculate a 10% late charge to enter in Box 5.
- Add any other payments in Box 6 and compute the total payment due in Box 7 by summing Boxes 4, 5, and 6.
- Sign the report in the certification section and indicate the title of the signer.
- Save your changes, download the completed form, print it, and prepare to submit it along with any payment.
- Mail the completed report to the Oregon Department of Transportation address provided, ensuring all records are maintained for three years.
