How to fill out the NV OHV-007 online

This guide provides a comprehensive overview of how to complete the NV OHV-007 application for a duplicate Nevada off-highway vehicle certificate of title online. Whether you are renewing your title or addressing a lost, stolen, or damaged one, this step-by-step guide will facilitate the process.

Follow the steps to successfully complete the application

  1. Press the ‘Get Form’ button to access the online version of the NV OHV-007 application. This will allow you to fill out the form in an interactive editor.
  2. Begin by entering the vehicle identification number (VIN), year, make, and type of the off-highway vehicle. The options for vehicle types include motorcycle, snowmobile, all-terrain vehicle, side by side, dune buggy, and others.
  3. Fill in the registered owner(s) or lessee's information accurately, ensuring that it matches the data on the latest certificate of title. This includes full legal names, Nevada Driver’s License numbers or identification card numbers, dates of birth, phone numbers, and both physical and mailing addresses.
  4. If there is an owner #2, make sure to repeat the process for entering their information as specified in step-3.
  5. Complete the legal owner section if applicable, providing the lienholder or lessor's information, including their full legal name and address.
  6. If you wish for the certificate to be mailed to a different address, fill out the 'Requested By' section with the relevant details. Remember, a notarized letter of authorization is necessary for third-party requests unless it's a licensed Nevada OHV dealer.
  7. In the certification section, mark the appropriate box that describes the status of your original certificate (lost, stolen, or mutilated) and provide an explanation, if needed.
  8. Finally, once all sections are completed, you will need to notarize the application. Make sure that the notary stamp and public or authorized representative's signature are included.
  9. After completing the form, save your changes, download a copy for your records, and prepare to mail the form along with the required fee to the DMV OHV Section. Alternatively, you can bring it to a licensed Nevada off-highway vehicle dealer.
