2. Review the application requirements to ensure that you qualify to file this application. You must have received grades in a previous degree program at Baruch College and not be enrolled in another degree program.
3. Fill out the 'Student Information' section, providing your last name, first name, middle initial, and date of birth. Supply your CUNYfirst ID (EMPLID) or the last four digits of your Social Security Number.
4. Indicate your current street address, city, state, and zip code. Ensure that you are providing accurate information for contact purposes.
5. Answer the question regarding attendance at any school since leaving Baruch. If applicable, provide details about the school(s) attended and the dates of attendance, noting that official transcripts are required.
6. Read the 'Application Agreement' carefully. By signing, you certify that all statements on the application and supporting documents are true.
7. Sign and date the application.
8. After completing the form, save any changes, download the final version, and prepare to submit it via email to Reentry@Baruch.cuny.edu. Remember that applications sent by fax will not be accepted.
